The form on the website is sent directly to CAMS, there's no facility for the club issued licences on it, and no need for a club rep to fill anything out. The forms CAMS sends to the club have the "Temporary" slip on them. When you rock up to one of these days, or any CAMS sanctioned event, as a competitor they should ask for both your CAMS Licence and your club membership card ( to show you are a member of a CAMS affiliated club).
