Changeing 4 Stud To 5 Stud

I'm sure all of you with N/A skylines are in the same boat. I really want to do a 5 stud conversion. I found this on just jap...

and was wondering if anyone has tried one or know which one to get. There isn't a specific skyline one, but they may be some what universal. Any info would be great.

Also, how do they exactly work. Are they effectively a spacer with five studs holding onto your four studs. Cause I don't really like that idea, or do you change the whole this. The picture is confusing me.
