Its basically you connect the round bit to the neg side of the battery (I.E BLACK SIDE) and then bolt the wires onto anything special I.E engine, chassis etc. Really only used if you have idle issues or have a big sound system. Oh and of course if you have an issue with starting etc. Even 1000hp cars dont use them so ...

Actually, I would like to give that a go as I forgot to put the earth wire back on between the body and exhaust manifold once and could tell the car wasn't as smooth and seemed a little slower under 3,000rpm. Also just a little harder to start. So yeah, if anybody out there has back to back comparisons with a full earthing kit I'd like to know.
