A. He didn't care whether he was in a dream or reality, was just happy to see his kids faces and that made him content with whatever he had in the end, dream or reality. Remember his wife's riddle? "You're waiting for a train...you don't know where it goes but this doesn't bother you because you're together etc."

Or was he?

B. He didn't need the spinning top totem to tell him it was reality, seeing his kids faces was a totem in itself because he knew he could never see their faces in a dream no matter how hard he tried.

Or did he?

C. His kids faces at that point were more important than discovering whether he was in a dream or reality, hence they distracted him. He probably spun the totem again later to check lol.

he dreams and dreams in a dream and then dreams in the dream he is dreaming...

Everyone is alive at the end and the last scene is of him dreaming in the dream of the dream (3rd Dream) of him and his kids.
