You can do it with head on but you need to pressurize the cylinder so the valve don't drop... Kent of a job!

In other words I'm probably right. Yes, technically you can do it with the head on, but who does?

Lots of people do. The techniques are as old as overhead valve engines. The oldest one is to push a length of rope into the cylinder through the sparkplug hole. Works fairly well, but a bit fiddly. You can actually buy the equipment you need to put compressed air in to hold the valves up specifically for the purpose, but people also use leakdown testers and self-built stuff. If you have a $200 head gasket or don't want to dismantle the top half of your engine, it is a perfectly valid way to avoid some pain (at the cost of a little extra pain in doing the seals themselves.)
