DIY Maintenance Questionnaire for FAQ - Your Input Needed

I'm preparing a self maintenance FAQ for the DIY section and need you guys to fill out the following form with AS MUCH info as you can give.

Basically it just asks you what you do to maintain your car both visually and mechanically, at what intervals, what products you use and where you take your car for servicing. Keep in mind that badmouthing businesses is not on, but you are more than welcome to give out reccomendations.

I'll be moderating this thread heavily, so no offtopic discussions. Copy and paste the following template to answer the questions. This will then be compiled into a FAQ so newbies don't have to post up a gazillion threads about how to maintain their new car.
